Resources, Principles, and the Practice of Health Equity

A compilation of training resources for community health care service providers in the Champlain region

This report presents a compilation of training resources on “health equity”. Healthy People 2020 defines health equity as “attainment of the highest level of health for all people. Achieving health equity requires valuing everyone equally with focused and ongoing societal efforts to address avoidable inequalities, historical and contemporary injustices, and the elimination of health and health care disparities.”

This list was created in order to support community health service providers in the Champlain Local Health Integration Network (LHIN) to ensure high standards of health equity throughout organizations, by ensuring that staff and board members who work at different levels of an organization are aware and sensitized on this topic.

THE CHAMPLAIN COMMUNITY HEALTH CENTRE NETWORK

Mandate

The Champlain Community Health Centre Network (the Network) is designed to increase collaboration and partnerships related to the delivery of primary health care services in the Champlain region. The Champlain CHC Executive Directors meet regularly to focus on strategic priorities and to build skills and connections. 

 This group of dedicated Executive Directors work together strengthening and enhancing the CHC Model of Care. The CHCs collaborate to improve internal organizational functions and to influence and shape policies of government and other external authorities. The Network undertakes this working partnership with the Champlain Local Health Integration Network (LHIN) and the Association of Ontario Health Centres (AOHC). 

 

Membership

The Network consists of 11 Community Health Centres within the Champlain region.

The Champlain CHC Network is a separate entity from the Coalition of CHRCs of Ottawa but there is overlap in membership. The membership of the Network and the Coalition is illustrated below.

 The five participating members, unique to the Champlain CHC Network, are located across the Champlain region. Each centre can be contacted directly through their website.

 The CHC Model of Care

CHCs focus on the social determinants of health. Inter-professional teams respond to people’s individual health issues. They also create programs and services that can reduce social and environmental problems and address harmful factors affecting community health. All services are carefully tailored to respond to the diverse needs of the many different communities being served.
